Do you currently do any other sports? Please list them in order of priority to you, including running.
What are your running goals? These can be general e.g. Improve running performance in all aspects, bring up weaknesses, increase endurance. Or more specific e.g. training for a particular event/race, or improve 5k time.
If known, what is your best running time for 800m (track only), 1500m (preferably on a track), 5k (road or track), 10k (road or track), half marathon, marathon. Please include the date which you achieved these and if average heart rate data is available for each please include this too.
Roughly, on average, how many hours per week do you have available to train running? This can be a range e.g. 3-6 or 15-18:
Do you have access to gym equipment and do you want to have strength and conditioning included in your plan? (We will discuss further during the initial call)
Do you own a treadmill? If yes what is the make and model?
Do you own a heart rate monitor chest strap? If yes what is the make and model and what is your maximum heart rate (that you could hit on most days if you did a max effort, not your all time peak heart rate)
